From the xCenta Dashboard, click on the 'Incidents' icon in the Modules section.



A new page will appear. Click on 'Create New Incident' button.



A new tab will appear. Fill in the required details and also the optional details if necessary. Selecting incident types and person categories will control the fields that appear to the right. For example, a Personal Injury Incident Type and an Employee Person Category will display the options below. Once you have filled in the relevant information click the 'Next' button.



A second page of the tab will then appear. For personal injuries an injury picker will appear on this page, this allows you to select the body part of the injured person and fill in the injuries that occurred. More than one injury can be entered, but it must be done one at a time. Once you have entered the injury(s) click the 'Update' Button and then click on the 'Next' button. 




The third page that appears is the Accident Record form, this is a required form that you need to fill in regarding the incident.

Once you have filled the required fields click the 'Next' button.



The fourth and final page will be the form wizard. Click on the relevant question. Either 'Did anyone witness the incident that isn't created as a user on xCenta?' or 'Does this incident require accident investigation?'

Once you have selected the relevant question(s), you can select whether you'd like to 'Open' the forms.


Click on the 'Create' button to complete the incident.




  


The Accident Investigation Form is to assist individuals in what relevant details need to be taken following an incident.


The Accident Record is required by law to be fully populated following every incident.


The Witness Statement is to be populated with accounts of what happened by individuals that saw the incident occur.


The HSE Personal Injury (F2508) RIDDOR Form is to be used when the incident becomes RIDDOR reportable, please click here for guidance on what is RIDDOR reportable.
