From the xCenta Dashboard, click on the Incidents icon in the Modules section.
A new window will appear. Click on the incident you wish to add a form to, for example INC-01059-1 and click on Add Form.
A new tab will appear.
The Form Template is based on the incident type so it will only present the user with the relevant forms.
The Form Display Name is the name of the form you have selected in the drop-down box above. You do have the ability to change it if needed.
The Deadline Date indicates when the form needs to be completed by.
Allocate to Users allows for a user to be assigned to a form, for example, if an employee saw the incident they can be assigned a witness statement.
Notifications is if the creator of the form would like to be notified upon completion.
Click Add Form once completed.
